Refreshes the outstanding-issues ledger snapshot after reconciling 42 queued inbox requests into the canonical tracker (per PR metadata). The snapshot’s ledger_revision SHA/timestamp moves forward, open count drops 121→102, and resolved rises 501→525, with matching P2/P3 count adjustments.

The diff removes many closed/archived rows from the snapshot’s open list (RAG denylist, retrieval RPC, medication status, Caring Contacts tasks, UI/CI nits, etc.) and adds new tracked items around release-browser-matrix (70m timeout cancelling main CI vs. prior 15 Firefox/WebKit failures), ledger archived-row correction gaps (#686WHW follow-up), Lighthouse baseline refresh policy, and audit remediation leftovers (#4BE39H).

#W98GR7 is materially rewritten: offline audit refutes the “delete-before-embed” enrichment loss model and documents what was actually shipped (repair migration, operator script, health check).
