Add AGENTS.md files to guide AI coding assistants - #9221
This adds the layered AGENTS.md files proposed in #9199: a root file that
routes, plus a short leaf file in each subpackage that has real,
package-specific traps (transforms, networks, data, losses, metrics, bundle,
apps, auto3dseg, engines, csrc, tests, docs).
The root file keeps to repo-wide material: what MONAI is, a repository map,
setup and test commands (only ones actually run in this checkout), and the
conventions from CONTRIBUTING.md that AI assistants most often miss (PRs
target dev, DCO sign-off with git commit -s, optional dependencies only
through optional_import, new public APIs also go into docs/source/*.rst).
Detail lives next to the code it describes, so a leaf file only exists where
it says something an assistant would otherwise learn the hard way.
It also updates .coderabbit.yaml, as suggested in the issue discussion, so
reviews check that changes to the code are reflected in the agent files: a
new path instruction covers AGENTS.md files themselves, and one for monai/** asks reviewers to verify the closest AGENTS.md was updated in
the same change when behavior, conventions, or public APIs move.
Local checks in a fresh CPU-only environment: ./runtests.sh --setup, ./runtests.sh --autofix, and ./runtests.sh --ruff pass, and python -m pytest tests/utils/test_look_up_option.py -q passes (8 tests).
Prepared with AI assistance, as labelled here per the Tool-Assisted
Contribution Policy in CONTRIBUTING.md. Happy to adjust the scope (for
example, root file only, leaves in a follow-up) if that is easier to review.
Types of changes
Non-breaking change (fix or new feature that would not break existing functionality).
Breaking change (fix or new feature that would cause existing functionality to change).
Quick tests passed locally by running ./runtests.sh --quick --unittests --disttests.
In-line docstrings updated.
Documentation updated, tested make html command in the docs/ folder. (Not applicable here: the new files live outside docs/source, so they are not part of the Sphinx build, and no documentation sources were changed.)
this adds a root AGENTS.md and short leaf files in the subpackages
that have real, package specific traps, as proposed in Project-MONAI#9199.
the root file stays general: project overview, repository map, the
commands, and the repo wide conventions from CONTRIBUTING.md.
detail lives next to the code it describes.
also updates .coderabbit.yaml, as suggested in the issue discussion,
so reviews check that code changes are reflected in the agent files.

Walkthrough
The pull request adds repository-wide and package-specific AGENTS.md guidance. The guidance covers project structure, commands, coding practices, documentation, package conventions, and tests. It updates CONTRIBUTING.md and adds path-specific review instructions for AGENTS.md files and changes under monai/.
The guidance and contribution updates present no identified merge-blocking risk.

Explanation
The title "Add AGENTS.md files to guide AI coding assistants" matches the main change. The PR adds a root AGENTS.md and package-level AGENTS.md files, plus a .coderabbit.yaml update and a CONTRIBUTING.md edit. The title is specific and concise.
Full details: Description check
Explanation
The description follows the template. It links the issue (Fixes #9199) and has a Description section that explains the root file, leaf files, the .coderabbit.yaml update, and the local checks run. The Types of changes section is filled in, with the Non-breaking box checked and the docs box annotated as not applicable. The unchecked items are not claimed as done, so the template is complete. Integration and quick test items are unchecked, and the description states which checks ran locally.
Full details: Linked Issues check
Explanation
Directly linked issue #9199 has coding requirements: a root AGENTS.md, short leaf AGENTS.md files where code-verified traps exist, links to existing guidance, a CONTRIBUTING.md update, and maintenance of the guidance. The reviewed head covers these. The root AGENTS.md (+101 lines per the summary) holds the project overview, repository map, verified commands, and conventions, and its table links to the package files. Leaf files exist for docs, apps, auto3dseg, bundle, csrc, data, engines, losses, metrics, networks, transforms, and tests. The CONTRIBUTING.md 'Tool-Assisted Contribution Policy' section at head now tells contributors to read the root and nearest package AGENTS.md and to update the applicable file in the same PR when a documented convention changes. This resolves the prior FAIL, which rested on the missing CONTRIBUTING.md change. The .coderabbit.yaml path instructions add guidance-maintenance checks as the issue discussion suggested. The remaining issue items (fresh-checkout command verification and testing with an AI assistant) are non-coding and are not assessed. Limitation: the changed-file diff could not be read, so leaf-file content rests on the supplied summaries and the CONTRIBUTING.md head blob.
Full details: Out of Scope Changes check
Explanation
All changes map to #9199. They are the root AGENTS.md, the package-level AGENTS.md files, the CONTRIBUTING.md paragraph directing assisted work to those files, and the .coderabbit.yaml review instructions for AGENTS.md maintenance. The PR objectives report that the .coderabbit.yaml changes support the issue's guidance-maintenance goal. No unrelated code, test, or configuration change appears in the supplied summaries. Limitation: the whole-PR text diff was unavailable, so this rests on the summaries and the CONTRIBUTING.md head blob.

Thanks for the review. All four comments are addressed in e882953:
AGENTS.md: the --autofix description now lists Ruff as well.
monai/csrc/AGENTS.md: --clangformat is described as formatting in place, not a read-only check.
monai/metrics/AGENTS.md: removed update from the cumulative metric lifecycle; accumulation is through __call__.
monai/transforms/AGENTS.md: the state guidance is narrowed, so RandomizableTransform can keep sampled parameters for the current call and for randomize=False reuse, while sharing one instance across concurrent calls is still called out.
(Comment posted with AI assistance, as noted on this PR.)
Document the synchronization rule in CONTRIBUTING.md.
CONTRIBUTING.md does not mention AGENTS.md or require contributors to update it when conventions change. The new rule in AGENTS.md:99-101 is therefore invisible to contributors who follow only the contribution guide.
Suggested fix
Code review and analysis can be assisted through the use of tools, for example linting or formatting tools. Diagnostic output from such tools does not form part of the submission but should be clearly labelled. The key emphasis is that these tools assist and do not replace human review, so contributors and reviewers must consider what might be missed by these tools, or what might be hallucinated. It might be helpful to review code with a different tool from one used for generation.
