Bug Description

It seems that the diagnostic date is not implemented.

Reproduction Steps

1. Process a DD with a diagnosis date
2. No change in the UI
3. Case entity has no diagnosis date

Expected Behavior

Implement diagnosis date.

This was implemented initially for sample reports and the value was extracted from the relevant sample report. Currently this has been disabled due to having the option to create Phone notifications.

We should change this back to as it was before being extracted from the sample report. Also Treatment Started should be reverted to the same mechanism.

Ideally the Phone notifications should be stored as sample reports see #13754.
As a temporary measure move Treatment Started on the Case entity because Therapy might be restricted based on access rights.

Diagnosis Date will be automatically determined based on the sample report and it will be disabled in case of creating a Phone Notification.
