Upgrade Mermaid to 11.12

Currently we're on 11.2 from Sep 9, 2024. There are quite a number of new features and fixes since then, which you can see here: https://github.com/mermaid-js/mermaid/releases.

This issue is a JavaScript conflict. See the issue for an easy workaround.

Side note: After multiple episodes of mermaid charts breaking when re-rendering large documents, especially when I update Quarto, I decided to quit using mermaid in favor of using simple front-ends to graphviz as detailed at https://hbiostat.org/rflow/rformat#sec-rformat-diagrams . My documents and presentations are far more robust as a result.

Unfortunately it's not consistent with tabs, so I have an annoying tradeoff between extremely long documents or rendering diagrams manually with no version control :/
